my name is Robert and unfortunately I don't speak slovensky or cesky. But I hope you can help me in english too

I tried to read this entry of my great grandfather Jakub Lez. But I still have problems with some words - see attached files.

One the first one, just one word is missing - I read the following:
Lež Jakub ?? z Pac-
ov á 10., manžel Magdalena nz.
Kopejtko nz z Křenice.
manželskí 30 let.

The second one is the cause of death and name and title of the priest:
I think I can read for now:

middle:
